The Hospice Aide is responsible for providing patient care (bathing, dressing, assisting with transfers) under the supervision of the RN Case Manager, as well as documenting the care performed.

Qualifications:

• Meets the training requirements of the State of Wisconsin. Must possess a current license to practice as a Certified Nursing Assistant in the State of Wisconsin. Ability to read and follow written instructions and document care given.

• Satisfactory references from previous employers.

• Is self-directing with the ability to work with little direct supervision. • Secure with issues of death/dying. • Provides a calm manner when in a patient’s home. • Has empathy for the needs of the ill, injured, frail and the impaired. • Demonstrates tact, patience and good personal hygiene. • Licensed driver with automobile that is insured in accordance with organization requirements and is in good working order.

Experience and Education:

• 1 or more years of experience. Education must be High School Diploma or Equivalent • Must be licensed as a Certified Nursing Assistant by the State of Wisconsin

Knowledge and Skills Required:

• Customer and Personal Service (This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction.) • Good oral and written communication • Good organizational skills.
