team. We are seeking compassionate Registered Nurses (RNs) to join our hospice team in full-

In this role, you will be responsible for creating and managing individualized care plans,
delivering skilled nursing care, and supporting patients and families through the end-of-life
journey. Our ideal candidate brings both clinical excellence and heartfelt compassion to their
work.
Key Responsibilities
- Coordinate with the interdisciplinary team to assess, plan, implement, and evaluate
individualized plans of care - Perform skilled nursing visits and symptom management in accordance with the hospice
plan of care - Provide education and emotional support to patients and their families
- Accurately document patient care and report changes in condition to the clinical
supervisor - Ensure continuity and quality of care through effective communication with all care team
members
Qualifications
- Active Registered Nurse (RN) license in the state of Wisconsin
- Prior nursing experience required; hospice or home health experience is a plus
- Strong communication, observation, and critical thinking skills
- Compassionate, patient-centered approach to care
- Valid driver’s license and reliable transportation
